Mission: To fund initiatives that address the health, education and well-being of individuals, families and communities.

Deadlines: Proposals will be considered at two times during the calendar year. Proposals are accepted from January 1 until May 31, and from June 1 until October 31. Proposals in the first round will be discussed, voted on and awarded by June 30. Proposals in the second round will be discussed, voted on and awarded by December 31. Proposals that are not awarded funding may be re-submitted at the discretion of the Trustees.

Requirements: Applicants must furnish proof of 501(c)3 non-profit status. Proposals must include a complete description of the project, program or position for which funding is sought and the rationale for such project, program or position (not to exceed three pages); a budget and estimated timeline for use of funds; an list of other potential or secured funding sources for this project, program or position.
